UCSD Admissions Information Request (Formerly Supplemental Review Questionnaire)

UC San Diego sent out “Admissions Information Request” (used to be “Supplemental Review Questionnaire”) on Thursday, January 4, 2024 but later sent a revised request (to at least some, if not all, of the original request recipients). Read on for general advice and how to get more help.

The original form sent by UC San Diego appeared to be the same questionnaire as last year, with questions about educational preparation programs, community involvement, and personal circumstances (you may answer only the questions that apply to your situation). The revised request (the examples I’ve seen) appeared to be more personalized. Make sure you take the time to carefully think through and respond to the questions. This is likely the final piece of information UCSD needs to make the admission decision..

If you received a questionnaire, you can forward the questions to me at and I will send you my interpretation of what the campus is looking for in your response. Please check your junk/spam folder for my reply. Keep in mind that the free guideline will be somewhat general in nature, since how you answer the questions will largely depend on your personal situation.
